“嘿,你确定还能工作吗?”急诊室顾问医生说这话时看我的眼神就像是看将死患者的家属一样。
“hey, are you sure that you still can work?” the ICU consultant asked me with worry, seemed like talking to the family of a dying man.
如果不是被紧急召回值班,这么冷的天我早就经钻进了被窝,也不会眼部感染。现在再临时通知别人来顶替也不太可能了。
If I was not emergently called back to ICU, I would already crawl into my bed and unable to have an eye infection. But it is unlikely to temporarily call anyone to replace me now.
我负责的是重症监护室,这里躺着医院里病情最严重的12位患者。从晚8点到早9点,他们由我负责。
I am in charge of the ICU, where 12 patients in most severe condition in this hospital stayed. From 8 pm to 9 am, I am responsible to take care of them.
第一晚是最困难的。因为要了解这些患者和他们复杂的病情,还有一些患者已经在医院住了几个月了。我要翻阅他们的病例记录,电子笔记,血液检查、X光扫描的数据,还要熟悉患者目前的生理状态。做这些的时候不断有护士打断我:患者转诊、急救室求助等。只有一直不停地喝咖啡、扑热息痛(Lemsip)和耐心才能挺过整个夜晚。
The first night is often the most difficult. Cause I need to know about these patients and their complex condition, while some patients have been being hospitalized for several months. What I need to do is not only to read their case notes, eletronic notes, results of routine blood examination and X-ray examination, but also to be familiar with their present physical conditions. And nurses will keep interrupting me with situations like patient transfer, emergency room asking for help and so on. Only by continuously drinking coffee and Lemsip, and being patient, can I get through a whole night.
今晚竟然有四个空床位,以前可从没有过。
Unbelievably, 4 vacant beds are left tonight, which neve happened before.
星期二 8pm-9am
Tuesday 8pm-9am
昨晚空出的四个床位中有三个白天的时候已经有人住进来了。
Those 4 vacant beds last night are only left one vacant now.
这时,一位家庭医生推荐来了患一位慢性肝衰竭的女性,她的病情在不断恶化,需要密切关注。而我们只剩下最后一张床位了,是否把这个床位给她让我很为难。生和死的决定往往是简单直接的,但是这些“擦边球”却很难决断。ICU是一个有限的资源,必须尽量为更多患者带来最大利益。我打电话给顾问医生咨询,她和我一起来见了患者,我们决定把最后一张床给她。
Recommanded by a family doctor, a lady who had chronic hepatic failure came. Her illness was getting worse, so we needed to keep an eye on her. We only had one last bed, whether giving her this bed made us feel awkward. The decision between living and death is usually simple and direct, while things like this at the cutting edge is not. ICU is a limited resource, so it should bring the biggest benefit to more patients. I called the consultant, together we met the patient, finally we decided to give the last bed to her

我的眼睛还没好。一些患者察觉到了一样,都小心翼翼地看我,好像我是幽灵一样。
My eyes didn’t recover yet. Some patients seems to notice this, looking me with carefulness like I am a ghost.
旁边房间里的那位男士情况不太好,他的器官都已经衰竭了。他没有亲人,最亲的人是他的邻居。凌晨三点的时候,这位邻居站在他的病床前,表情很是沉重,因为我的顾问医生说:我们已经尽力了。没过几分钟他就去世了
For organ failure, the man in the neighbor room was not in a good condition. He had no relatives, the only one he had was his neighbor. At 3 pm, this neighbor stood before his bed, looking gloomy, because my consultant told  the neighbor that we have done our best. Within a few minutes, he passed away.
我走到另外一位患者那里查看情况,回来的时候,那位邻居已经离开了。护士让我确认患者死亡。连悲伤都是无声的。如果我不能在情感上让自己离他们远一点,那就真的没办法继续做这份工作了。在确认他已经没有生命体征的时候,我停下来回忆了他曾在世的情景,希望他能得到安息。
After I came back from anothe patient, that neighbor has already left. A nurse let me sign on the death warrant. Even sorrow are soundless. If I can’t keep myself far away from those patients in the mood, then I am really no longer able to do this work. I stopped to recall things about him, when I am sure that he has no vital signs, hoping that he could rest in peace.
星期四 8pm-9am
Thursday 8pm-9am
我们已经没有床位了,但还要收两位患者。因此,所有这些重症患者中状态“最好”的两位就被转移到了手术恢复室(theatrere covery),好腾出地方给情况较为严重的两位患者。他们在恢复室也会得到很好的照顾,但是肯定比不上在ICU。在供不应求的情况下,这种事经常发生。有时候,如果医院没有足够的床位,患者会被转诊到其他医院。
We had no more beds, but there were still two more patients. Therefore, two patients in the “best” condition among all these critically ill patients were transferred to a theatrere covery, so we could make room for the two patients whose situations were more severe. Patients in the theatrere covery will be good care of but definitely not as good as in ICU. Things like this happen very often when demand exceeds supply. Sometimes, if a hospital do not have enough beds, patients will be transferred to other hospitals.
另外有三位患者出现了精神错乱,变得很激动,伸手要去摘掉身上的管子。仔细检查过后,我给其中两个人开了镇静剂以免他们伤害到自己。新转来的一位患者的家属已经到了,看起来非常担心。患者说自己需要休息。这是一个好的征兆:如果他还能说话就说明他正在好转。
Besides, three patients appeared deranged, became very emotional, trying to pull out tubes on their bodies. After carefully examining, I prescribed sadatives for two of them in case that they may hurt themselves. Familes of a new patient has arrived, seems to be very worried. The patient said that he needs to rest. This is a good sign—if he can speak, it is the sign that he is recovering.

Friday
早晨完成了交接,开车回家。接上女儿,带她去游泳课。这是缓解夜班疲劳最好的方法。
I finished the shift this morning and drived home. Picked up my daught and drove her to her a swimming class. This is the best way to reduce the tiredness of night shift.
最艰难的决定:是否接收肝衰竭患者。
The hardest decision: whether to receive patients with hepatice failure.
最满意的结果:看到患者病情不断好转回到普通病房。
The most satisfied result: seeing that patients are becoming better and finally get back to common ward.
